Medical Spreadsheet Templates– Template database of patients in Excel sheet that you can download for free. It is a simple Excel book that contains 2 sheets, one is the tab where we can individually view the data of each patient that has been entered in the database and the other sheet is where the data corresponding to each field is entered. The sheet has the following fields (which can be modified by another name): Surname, First Name, Address, Postal Code, Location, Province, Birth, Sex, Social No., Telephone, Date of consultation, Date High , Diagnosis, Treatment, Notes / Observations, if we wish we can add more fields.

In the tab sheet when entering in cell F2 the history number, Code, DNI or whatever, it will automatically show us the patient data that has been previously entered in the database.

All the fields that appear in the sheet can be changed by another name or add more fields in the database. If we have put a larger field in the database sheet and we want it to appear in the tab sheet, we will have to indicate which column of row 1 it is in, if it is column R, we apply the following

formula: = ‘ Data base ‘! R1 (we would see the name of the field in column R)

To show us the data of the field of the R column of the patient that has been indicated in cell F2 tab sheet, we apply in an empty cell of the tab sheet the following formula:

If the data of the field instead of being in column R (cell R1) is in column S (cell S1), we change in the formula the indicative of the column and it would look like this: =

On the tab page, if we position ourselves in the cells that contain data from the fields, we can see the formulas used and can help us if we want to enter more data in the tab sheet.

The dates in the column where the Date of registration field is, if it is less than the date of consultation, will appear with another format (in this case, a red letter), which can be changed if desired by selecting the Start-Sheet – Conditional Format
